Physicians Plus Insurance Corporation (Physicians Plus) is pleased to offer you secure access to your health insurance information online at your convenience using MyChart.

This Terms & Conditions statement outlines our practices and our sensitivity to your right to privacy. Your continued use of this site indicates your agreement to be bound by the terms and conditions set forth below. We reserve the right to revoke MyChart access at any time for any reason. Physicians Plus members may request account deactivation by calling Member Services at (608) 282-8900 or (800) 545-5015.

Access to Your Health Insurance Information: MyChart is available through a secure internet connection at mychart.pplus.com. This is a voluntary service, offered as a convenience and courtesy to users who have insurance through Physicians Plus.

Information Accuracy: You are responsible for ensuring that all information that you provide during sign up and your continued use of the MyChart System is accurate and true at the time you provide the information. Any deliberate falsification of information that you provide is a breach of these Terms & Conditions, and Physicians Plus reserves the right to revoke or suspend your access to MyChart and to take any appropriate legal action. After sharing your e-mail address, an activation code will be sent to the supplied e-mail address; it is your responsibility to ensure that only you have appropriate access. Physicians Plus is not responsible if others with the same address access the e-mail containing your activation code. Should we have any questions regarding your submission, we will contact you via the phone number or address submitted. It is extremely important that you keep your username and password completely confidential. Anyone with access to your username and password will be able to access your confidential health plan information. That person will also be able to read your messages and send new messages as if that person were you. It is your responsibility to prevent disclosure of your login and password and to change your password if you feel that its security has been compromised. Physicians Plus is not responsible or liable for disclosures of health insurance information obtained through unauthorized use of a user's password or the sharing or viewing of this information with or by individuals not legally authorized to do so. You may change your password at any time.

Protecting Login Information: Users will receive an activation code when they request a MyChart account. The code may be given to the user in person or mailed to the user’s address on file. The activation code is used to gain first-time access, at which time a permanent user ID and password are created by the user. It is extremely important that users keep the activation code, user ID and password confidential. Anyone with access to this information will be able to access the user’s MyChart account. I understand that Physicians Plus has no liability for consequential damage arising from a breach of information resulting from the sharing or compromise of my user ID or password, for the loss of any password or for misuse of my password or other Physicians Plus privileges. I hereby attest to the accuracy of the information I have provided when signing up for MyChart.

Response to Electronic Communication/Non-Emergency Use Only: The timeline for a response to your message may vary, from same day to two business days, based on the subject of your message. Users should allow at least two business days for a response. Accordingly, MyChart should not be used to communicate in emergency situations. In emergencies, please take appropriate actions, such as calling 911 or going directly to an emergency department. You agree that you will use the electronic communication privileges of the MyChart System in a courteous, respectful, efficient and informative manner and that you understand that this service is not for social dialogue. You are responsible for all information you send to the MyChart System. You may not post or transmit any information that is illegal, threatening, libelous, defamatory, obscene, ethnically or racially offensive or discriminatory, profane, invades the privacy of another person or any other information or materials that constitute a crime, give rise to a civil action or otherwise violate any law or confidentiality agreement. Physicians Plus reserves the right to refuse to post any such message, to revoke or suspend your access to MyChart and/or to take any appropriate legal action related to any inappropriate use of the MyChart System or the information it contains.

Receiving Information On-line: Once a Physicians Plus member activates a MyChart account, the user may opt to access health plan information online. This information may include, but is not limited to, benefit summaries, explanations of benefits, make premium payments and contacting Member Services.

Proxy Access: You can use this feature to access the MyChart account of someone other than yourself. You may request Proxy Access if you are:

  1. The parent or legal guardian of a minor child under the age of 18, or
  2. A legally appointed guardian or health care decision-maker for a patient over the age of 18

Physicians Plus reserves the right to collect, examine or make copies of any information it considers necessary to determine whether legitimate proxy rights exist and to deny eligibility if this information is lacking. Physicians Plus reserves the sole right to determine whether proxy eligibility exists and to whom it will grant Proxy Access rights.

When the patient reaches the age of 18 or obtains a court order removing the disabilities of minority, all Proxy Access will be discontinued and/or denied. At that point, the member is legally recognized as an adult in their own right and may request direct access to his/her own MyChart System account.

Secure Communications: All communications with MyChart users occur over a secure Internet Connection. If you provide your personal e-mail, then you have the option to be notified via email when there is new information available in your MyChart account. These e-mail messages, however, will not contain any confidential health plan information. All of the confidential health plan information available to you in MyChart is protected and securely maintained by Physicians Plus.

Web site Links: MyChart may offer links to related web sites. These web site link(s) are for informational purposes only. The information is not endorsed nor has the accuracy of the information provided by these web sites been verified, and patients should not rely on any of the information found on the web sites for purposes of treatment or diagnosis and should seek medical assistance as appropriate.

MyChart may provide links to other websites that are not owned or controlled by Physicians Plus. Please consult the privacy statement of each web site you visit.

Web site Changes and Availability: Physicians Plus will support, change or improve its MyChart service as it deems necessary to meet the needs of its members and to comply with applicable laws and regulations. Although Physicians Plus intends to offer the MyChart System services 24 hours per day, 7 days per week, 365 days per year, there will be times when MyChart becomes unavailable due to necessary maintenance, system changes or a technical malfunction. Physicians Plus will employ commercially reasonable practices to maintain the MyChart System service availability. Physicians Plus will provide support services to assist you in your use of the MyChart System. Please call (608) 282-8900 or (800) 545-5015 if MyChart is unavailable. Updates or changes to the MyChart web site may occur periodically. The web site may not always be available due to system maintenance, backups or other system events.

Your Agreement: By accessing or using MyChart, you agree to be bound by the terms and conditions as stated above. If you do not agree to these Terms and Conditions, do not proceed with sign up or use of this web site. Physicians Plus reserves the right to make changes to these Terms & Conditions at any time and for any reason. When you use MyChart, you agree to all terms and conditions of the posted MyChart Agreement, which are in effect on the date that you access and use the MyChart System. If you find that the current Agreement is unacceptable to you, you must stop using the MyChart System and terminate your MyChart System enrollment immediately.
